On Mundoo Island, Martin spends a day cattle herding; then he heads to Phillip Island, which boasts one of Australia's greatest conservation success stories, despite being a popular tourist destination. Martin also visits Maria Island and works with scientists to learn about the disease threatening Australia's iconic Tasmanian devil.

Martin begins his journey in Sydney's iconic harbor, then travels to Lord Howe Island, where he learns about the Kentia palm, the world's most popular indoor plant. When visiting Norfolk Island, Martin learns the brutal truth about the most notorious prison island in the British Empire.

Sutton directs his team to prioritize the search for a white van, which he believes will lead them to the killer, but morale dwindles when the hunt proves daunting. Sutton's obsession with the case puts strain on his relationship with his wife when he decides to cancel on a family trip at the last minute.

Martin enjoys salsa dancing lessons and rum tasting in Puerto Rico before heading to the Sea Islands off the coast of Georgia, where the inhabitants attempt to preserve the traditional practices of African slaves. On the Outer Banks of North Carolina, Martin learns about the mysterious lost colony of Roanoke, before traveling to Chincoteague, Virginia, to watch the world-famous wild pony swim.

Martin begins his journey in Hawaii, the only US state made up entirely of islands. After leaving tourist-filled Oahu, he explores the active volcanoes of the Big Island, the largest island in all US territory, and the ancient traditions of Molokai Island. Next, Martin heads 2,500 miles north to Kodiak Island in Alaska, where there is one bear per square mile.

Doc Martin makes a bad impression on one Portwenn resident before he's officially hired. Things go downhill from there after he casts a pall over a local festival and gets punched in the nose.

Doc Martin is appalled to learn that Louisa wants her old job back as head teacher, and Aunt Joan is having money problems.

A midwife arrives in Portwenn and immediately clashes with Martin, while Bert Large is smitten with a new cook at his restaurant.

(Aired in the U.S. as "City Slickers") A family of city slickers moves in next to Louisa, bringing modern ideas about parenting and their bratty son. Doc Martin learns that the constable is agoraphobic as well as narcoleptic.

It's Doc Martin's last day in Portwenn, and his office is packed with patients. Edith turns up, expecting an apology, and Aunt Joan rebukes Martin as well.
